NHTSA Complaint about BACK OVER PREVENTION: CAMERA SYSTEM filed August 28, 2021

NHTSA complaint #1765492 (ODI reference 11430864) concerns a 2020 FORD EXPLORER and was filed on August 28, 2021. The owner reports the failure occurred on August 27, 2021. The report was geocoded to Illinois based on the filer's self-reported location. The affected component is categorized as back over prevention: camera system, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.

The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.

Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same FORD EXPLORER cohort independently describe similar back over prevention: camera system failures, defect investigators have grounds to open a PE and request manufacturer data. Allegedly they replaced the back-up camera. On 8/27/2021 upon returning home from an approximately 3 mile drive, the back-up camera again failed while backing into our driveway. The DIC displayed the message "Back up assistance unavailable." While on vacation in Colorado, the back-up camera failed every day for 2 weeks (8/01/2021 - 8/13/2021). The failure would occur after extended drives at highway speed. On one occasion, the back-up display flickered between the image, blackout, and blue screen. On one occasion both the backup display and the overhead 360 display, displayed only blue screens. Turning off the vehicle and leaving it sit for 30 minutes or more clears the problem. The dealer claims they are unable to reproduce the problem. I have repeatedly told them that the vehicle must be driven for the problem to pre
