ON CERTAIN TRUCKS EQUIPPED WITH A SAF HOLLAND SIMPLEX FIFTH WHEEL, THE LEVER ARM PIVOT BOLT MAY HAVE BEEN OVER TIGHTENED. AN OVER TIGHTENED PIVOT BOLT CAN DISTORT THE PIVOT BOLT SPACER SLEEVE, WHICH MAY PREVENT THE FIFTH WHEEL LATCH JAW FROM CLOSING AND LATCHING SECURELY AROUND THE TRAILER KING PIN

2008 MACK SEMI, UNIT 1 AND UNIT 2 WERE EASTBOUND ON THE OHIO TURNPIKE. UNIT 2 STATED ICE FROM UNIT 1 TRAILER FLEW OFF HIS VEHICLE STRIKING UNIT 2 WINDSHIELD. UNITS MOVED FURTHER UP THE ROAD FROM AREA OF IMPACT. NO ICE WAS FOUND ON THE ROADWAY. OFFICER STATED, HE CHECKED UNIT 1 FOR ANY SNOW/ICE ON THE TRAILER AND SAW NONE FROM THE INSPECTION FROM GROUND LEVEL. *BF (OHIO TRAFFIC CRASH REPORT #10-0098-91)*JB UPDATED 03/29/11 *BF
