NHTSA Complaint about FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL filed December 24, 2025
NHTSA complaint #2160302 (ODI reference 11706979) concerns a 2023 TESLA MODEL Y and was filed on December 24, 2025. The owner reports the failure occurred on November 21, 2025. The report was geocoded to Illinois based on the filer's self-reported location. The affected component is categorized as forward collision avoidance: adaptive cruise control, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.
The filer flagged the following severity indicators: crash: yes, fire: no, injuries: 0, fatalities: 0. A complaint that flags a crash, fire, or fatality is escalated on NHTSA's internal review queue and factors more heavily into any Preliminary Evaluation decision on this make and model. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.
Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same TESLA MODEL Y cohort independently describe similar forward collision avoidance: adaptive cruise control failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description
While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. Throughout this sequence, the vehicle remained unresponsive to braking and stop inputs. Vehicle data later confirmed that brake pedal application was detected prior to impact, yet braking response was insufficient to prevent conti
