Open-data reference.
NHTSA Complaint about BACK OVER PREVENTION: WARNINGS filed January 31, 2024
NHTSA complaint #1963252 (ODI reference 11569054) concerns a 2021 FORD EXPLORER and was filed on January 31, 2024. The owner reports the failure occurred on January 29, 2024. The report was geocoded to Michigan based on the filer's self-reported location. The affected component is categorized as back over prevention: warnings, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.
The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.
Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same FORD EXPLORER cohort independently describe similar back over prevention: warnings failures, defect investigators have grounds to open a PE and request manufacturer data. Related filings for the same vehicle and component appear below, and the detail page for the full 2021 FORD EXPLORER shows the complete component-level complaint distribution alongside any active investigations or recalls.
Complaint Description
My complaint relates to: Manufacturer Recall Number: 23S23 NHTSA Recall Number: 23V342 INTERMITTENT EXPERIENCE OF A REAR CAMERA BLUE ON THE SYNC SCREEN WHEN THE VEHICLE IS PLACED IN REVERSE and notification that rear parking assistant is disabled. Per January 2024 I received a notification that a remedy is available (replacement of rear camera and software update of SYNC). The dealership was not able to perform the remedy as they told me that the Accessory Protocol Interface Module (APIM) showed error codes and could not be updated. I was told to replace the APIM on my own expenses of ~$1,800. I am not willing to pay for a hardware replacement as I strongly believe that the APIM was defected by two prior updates: 1. prior effort of Ford to solve blue screen issue via software update and 2. a subsequent botched Over-The-Air update to version 22251 which is know to having bugs and causing APIM issues according to various Internet forums: [XXX] [XXX] [XXX] [XXX] So I have a securit
