Since my original lease of my 2015 Toyota Yaris SE from Hanlees Hilltop Toyota in January 2015, both the front and rear bumpers have continually come unfastened. This was described as a known defect by two employees at this location: Tito Anopol (phone [XXX] ) and [XXX] (phone [XXX] ). [XXX] noted the roof trim was also notorious for coming unfastened. He removed the top inlets from the roof exterior, helped replace them with OEMs, yet they still flew off the vehicle on the freeway. On August 29, 2023, the service team at Hanlees Hilltop Toyota verified the tabs on the bumpers are broken, and are even saying they cannot guarantee they will be able to properly install new parts if I order them from here, and are trying to refer me to a body shop? THE FRONT END WAS SEVERELY DAMAGED, BUT THE AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS DESTROYED AND TOWED FROM THE SCENE. A POLICE REPORT WAS FILED. THE DRIVER SUSTAINED FULL BODY AND FACIAL CONTUSIONS, WHICH REQUIRED MEDICAL ATTENTION. THE CAUSE OF THE NON-DEPLOYMENT OF AIRBAGS DRIVER OF FAILURE WAS NOT DETERMINED. INSTALLED FACTORY NEW CONTROL MODULE FROM TOYOTA, HAD NALLEY TOYOTA, ROSWELL, GA RESET THE AIRBAG. HALLEY DETERMINED THAT THE NEW CONTROL UNIT WAS DEFECTIVE AND REPLACED THE BRAND NEW UNIT WITH ANOTHER NEW UNIT. AT THAT TIME THEY DETERMINED THAT AN AIRBAG THAT DID NOT INFLATE ALSO HAD TO BE REPLACED. TWO DEFECTIVE UNITS IN AN AIR BAGS SYSTEM IS A POTENTIALLY FATAL SITUATION. REPAIR WAS DONE AT A BODY SHOP. I HAVE THE AIRBAG THEY DID NOT RETURN THE DEFECTIVE CONTROL MODULE. PART NUMBER 805736514P3K

THE TOYOTA 4 DOOR YARIS SUDDENLY ACCELERATED UPON INITIATING MOVEMENT FROM A RIGHT TURN STOP. THE VEHICLE ACCELERATED TO APPROXIMATELY 30 MPH WITHIN 100 FT. AND CAUSED A HEAD ON WITH FORD 250 TRUCK. UPON IMPACT, THE STEERING WHEEL AIR BAG DEPLOYED AND DRIVER OF THE YARIS SUSTAINED A CONCUSSION, FRACTURED STERNUM AND SEAT BELT BRUISING. THERE WAS SOME DAMAGE TO THE FORD 250 BUMPER AND THE TRUCK NEEDED REPAIRS INCLUDING A NEW BUMPER AND REALIGNMENT OF THE TRUCK VEHICLE TOTAL ALIGNMENT. IT IS UNKNOWN IF THE DRIVER OF THE FORD 250 SUFFERED ANY INJURIES. THE YARIS WAS TOTALED INCLUDING COMPLETE CRUSHING OF THE ENGINE WHICH ENDED UP NEAR THE DRIVER'S SEAT.
