During the crash, the front driver side seat belt failed and did not secure the driver and the driver side air bag did not deploy. VEHICLE REGISTERS DOOR ADJAR CONSTANTLY. CAUSES DOME LIGHTS TO STAY ON EVEN WHILE DRIVING, CAN'T LOCK DOOR, AND BATTERY GOES DEAD. THIS IS AN ISSUE! UNINTENDED ACCELERATOR FIRST HAPPENED APPROXIMATELY 18 MONTHS AGO. THE FAILURE MILEAGE WAS UNKNOWN.
