THE CONTACT STATED EACH MAT HAS TWO CLIPS THAT MADE IT DIFFICULT TO REMOVE THE MAT AND CAUSED THE CONTACT TO INJURE HER FOOT WHEN IT WAS EXPOSED. THE CLIP ALSO MADE THE MAT BULKY. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 4,000.
