TOYOTA IS RECALLING CERTAIN MODEL YEAR 2008 THROUGH 2011 LEXUS LX570, 2003 THROUGH 2009 TOYOTA 4RUNNER, AND 2006 THROUGH 2010 RAV4 VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

TL* THE CONTACT OWNS A 2009 LEXUS LX570. THE CONTACT WAS DRIVING 30 MPH AND ATTEMPTING TO STOP WHEN THE BRAKE PEDAL WAS ABNORMALLY DEPRESSED INTO THE FLOORBOARD. THE CONTACT STATED THAT ALL OF THE LIGHTS ON THE INSTRUMENT PANEL ILLUMINATED ACCOMPANIED BY A WARNING SIREN, INDICATING A BRAKE MALFUNCTION. THE CONTACT THEN ENGAGED THE SAFETY BRAKE AS THE VEHICLE COASTED IT TO A STOP. THE VEHICLE WAS TOWED TO A DEALER AND DIAGNOSED AS THE BRAKE PADS AND ROTORS BEING WORN AND A LACK OF BRAKE FLUID.THE CONTACT STATED THAT THE HE HAD BEEN KEEPING UP WITH THE RECOMMENDED MAINTENANCE OF THE VEHICLE. THE VEHICLE WAS LEFT WITH THE DEALER WHERE THE CONTACT WAS AWAITING REPAIRS. THE FAILURE AND THE CURRENT MILEAGE WAS 35,000. THE VIN WAS NOT AVAILABLE. UPDATED 02/15/12*BF UPDATED 03/12/12

I AM AN EXPERIENCED CPST (>800 SEATS PER YEAR) WHO YESTERDAY ASSISTED A MOTHER WITH INSTALLATION OF HER NEW GRACO SR-35 INFANT CAR SEAT W/ BASE MODEL # 1759703 IN HER 2009 LEXUS LX570 SUV. THIS COMPLAINT IS SPECIFIC TO A VEHICLE INTERIOR FEATURE THAT MADE IT VERY DIFFICULT TO CORRECTLY INSTALL THE CHILD RESTRAINT. THE VEHICLE BENCH SEAT IN THE SECOND ROW HAD LEATHER INTERIOR. THE DESIGN OF THE VEHICLE SEAT CUSHION INCLUDES A SUBSTANTIAL ROUNDED CUSHION THAT PROTRUDES FORWARD AT THE VEHICLE SEAT BIGHT. THIS SUBSTANTIAL THICKNESS OF MFG DESIGNED PADDING INTERFERES SIGNIFICANTLY WITH THE INSTALLATION OF AN INFANT SEAT BASE BECAUSE IT DISPLACES THE BASE OF THE INFANT SEAT SEVERAL INCHES FORWARD OF THE SEAT BIGHT. THE DISPLACEMENT OF THE BASE SEVERAL INCHES FORWARD OF THE SEAT BIGHT PROHIBITED SECURE INSTALLATION OF THE BASE WITH BOTH THE LATCH SYSTEM AND THE SEATBELT. TOYOTA SUVS OF SIMILAR BODY TYPE ALSO HAVE A SIMILAR SEAT CUSHION FEATURE HOWEVER IT IS NOT AS PRONOUNCED. ATTEMPTING

MY 9 YEAR OLD SON NEARLY STRANGLED IN FRONT OF HIS SISTER AND TWIN SIBLINGS (5 YEARS OLD) WHEN THE MIDDLE SEAT BELT OF THE THIRD ROW GOT TANGLED AROUND HIS NECK. THIS SEATBELT HANGS FROM THE CEILING. I COULD NOT DISLODGE THE BELT FROM AROUND HIS NECK, AS IT WAS NOT OBVIOUS HOW TO DISENGAGE THE CEILING LATCH. WE WERE VERY FORTUNATE TO STOP A PASSING CAR, RETRIEVE A PAIR OF SCISSORS AND CUT THE BELT TO FREE MY SON. HE WAS ON THE VERGE OF PASSING OUT FROM LACK OF OXYGEN. THE PARAMEDICS TOOK HIM THE TO THE ER, WHERE THEY DETECTED A RASH AROUND HIS FACE AND NECK THAT IS COMMON WITH STRANGULATION. HE WAS BADLY TRAUMATIZED, ALONG WITH THE REST OF US. I REPORTED THE INCIDENT TO LEXUS AND FOLLOWED UP NUMEROUS TIMES. EACH TIME THEY ADVISED ME THAT THEY WERE INVESTIGATING THE INCIDENT AND WOULD GET BACK TO ME. THEY NEVER DID DESPITE MORE THAN 6 EFFORTS TO SPEAK WITH SOMEONE. *TR

TL* THE CONTACT OWNS A 2009 LEXUS LX570. WHILE DRIVING APPROXIMATELY 35 MPH, THE VEHICLE ABNORMALLY ACCELERATED. THE GEAR SHIFT WAS PLACED INTO NEUTRAL AND THE VEHICLE STOPPED ACCELERATING WHILE THE ENGINE CONTINUED TO EXHIBIT UNUSUALLY HIGH RPMS. PRESSURE WAS APPLIED TO THE BRAKE PEDAL BUT THE VEHICLE WOULD NOT STOP UNTIL THE IGNITION WAS TURNED OFF. UPON EXAMINATION, THE CONTACT WANTED TO MAKE NOTE THAT THE FAILURE DID NOT INVOLVE THE FLOOR MATS. THE POLICE APPEARED ON THE SCENE TO ASSIST WITH PUSHING THE VEHICLE OFF THE ROADWAY. THE VEHICLE WAS NEITHER EXAMINED FOR THE CAUSE OF FAILURE NOR REPAIRED. THE FAILURE AND CURRENT MILEAGE WAS 20,000.
