SHE STATED THAT HER SEAT BELT LOCK SYSTEM FAILED, WHICH CAUSED THE SHOULDER BELT NOT TO RESTRAIN. THE LAP BELT WAS THE ONLY PART THAT PROPERLY RESTRAINED.
