Open-data reference.
NHTSA Complaint about SUSPENSION:REAR:SPRINGS:LEAF SPRING ASSEMBLY:U-BOLT, LEAF SPRING TO AXLE filed July 13, 2004
NHTSA complaint #484512 (ODI reference 10081001) concerns a 2001 WORKHORSE W22 and was filed on July 13, 2004. The owner reports the failure occurred on July 6, 2004. The vehicle had 16,367 miles on the odometer at the time of the incident. The report was geocoded to Mississippi based on the filer's self-reported location. The affected component is categorized as suspension:rear:springs:leaf spring assembly:u-bolt, leaf spring to axle, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.
The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.
Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same WORKHORSE W22 cohort independently describe similar suspension:rear:springs:leaf spring assembly:u-bolt, leaf spring to axle failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description
WE HAVE A 2001 DUTCHSTAR CLASS A MOTORHOME, BUILT ON WORKHORSE CHASSIS NUMBER 28321. I AM WRITING FOR A NUMBER OF REASONS. THE FIRST BEING THAT I AM REQUESTING INFORMATION ON POSSIBLE RECALLS THAT INCLUDE THIS UNIT. I UNDERSTAND THAT THERE ARE TWO CAMPAIGNS FOR THE 2001 WORKHORSE CHASSIS, NHTSA # 04V084000, HAVING TO DO WITH SERVICE BRAKES, HYDRAULIC AND # 01V317000, HAVING TO DO WITH SUSPENSION:REAR:SPRINGS:LEAF SPRING ASSEMBLY:U-BOLT, LEAF SPRING TO AXLE. I WOULD LIKE TO KNOW WHETHER OUR UNIT IS INCLUDED AND IF SO, WHY WE WERE NOT NOTIFIED. THE SECOND REASON I AM WRITING HAS TO DO WITH THE ABS BRAKE SENSORS ON THE FRONT WHEELS. IN OCTOBER OF 2003, WE HAD TO HAVE THE RIGHT BRAKE SENSOR REPLACED AFTER IT HAD BURNED UP WITH ONLY 13,523 MILES ON THE UNIT. ON JULY 6, 2004, BOTH FRONT ABS BRAKE SENSORS BURNED UP AND REQUIRED REPLACING WITH ONLY 16,367 MILES. LUCKILY, WE WERE TRAVELING AT A LOW SPEED AS THE LACK OF THE SENSORS APPARENTLY CAUSED BOTH FRONT BRAKES TO LOCK UP, W
