NHTSA Complaint about FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL filed February 16, 2026

NHTSA complaint #2177269 (ODI reference 11718284) concerns a 2025 TOYOTA GR COROLLA and was filed on February 16, 2026. The owner reports the failure occurred on September 1, 2025. The report was geocoded to Washington based on the filer's self-reported location. The affected component is categorized as forward collision avoidance: adaptive cruise control, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.

The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.

Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same TOYOTA GR COROLLA cohort independently describe similar forward collision avoidance: adaptive cruise control failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description

The car showed a message that there is a system error and many warning lights came on, including the check engine light. Most of the warning lights indicated that many of the car's ADAS functions were disabled. The car also switched from AWD to FWD. The error code turned out to be P2C90 which relates to the PCV system. So, the car turned off all the safety features as a result of a minor emissions issue! Toyota has no repair for this; they just clear the code, ignoring completely that the safety features are turned off until the error is cleared.
