Open-data reference.

NHTSA Complaint about BACK OVER PREVENTION: SENSING SYSTEM: CAMERA filed January 30, 2026

NHTSA complaint #2171510 (ODI reference 11714421) concerns a 2019 TOYOTA RAV4 and was filed on January 30, 2026. The owner reports the failure occurred on May 1, 2025. The report was geocoded to Utah based on the filer's self-reported location. The affected component is categorized as back over prevention: sensing system: camera, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.

The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.

Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same TOYOTA RAV4 cohort independently describe similar back over prevention: sensing system: camera failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description

Infotainment Center turns on and off, and will not work. Because the screen turns on and off, the camera for back up intermittently works. May 2025 i took it to Karl Malone Toyota in Sandy Utah. They said to buy a 3rd party, after investigation I confirmed that no display has the options to reset maintenance, change clock on the dashboard, and communicate with the car. Yes radio, navigation, carplay, etc work with a 3rd party system, but the functions that talk with the car do not work on any 3rd party system.
