NHTSA Complaint about BACK OVER PREVENTION: SENSING SYSTEM: CAMERA filed November 10, 2025

NHTSA complaint #2147538 (ODI reference 11698672) concerns a 2016 TOYOTA 4RUNNER and was filed on November 10, 2025. The owner reports the failure occurred on December 15, 2024. The report was geocoded to Arizona based on the filer's self-reported location. The affected component is categorized as back over prevention: sensing system: camera, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.

The filer flagged the following severity indicators: crash: no, fire: no, injuries: 0, fatalities: 0. No crash, fire, or fatality was associated with this report, which places it in the early-warning stream rather than the priority-review stream. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.

Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same TOYOTA 4RUNNER cohort independently describe similar back over prevention: sensing system: camera failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description

I have a 2016 Toyota 4Runner at 143,011 miles. When it hit 125,000 mile mark the backup camera stopped working intermittently. The screen in the car goes blank. Turning the car on/off or changing gears does not affect this issue. It’s random and seems to becoming more frequent as more miles are put on. I will take it in however when I called them they said since it’s not a known issue I will have to replace it and it’s not cheap!
