NHTSA Complaint about FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING filed April 10, 2025

NHTSA complaint #2081960 (ODI reference 11653934) concerns a 2024 TOYOTA RAV4 and was filed on April 10, 2025. The owner reports the failure occurred on February 13, 2025. The report was geocoded to California based on the filer's self-reported location. The affected component is categorized as forward collision avoidance: automatic emergency braking, one of NHTSA's standardized taxonomy codes used to group defect patterns across make, model, and year.

The filer flagged the following severity indicators: crash: yes, fire: no, injuries: 1, fatalities: 0. A complaint that flags a crash, fire, or fatality is escalated on NHTSA's internal review queue and factors more heavily into any Preliminary Evaluation decision on this make and model. Because a VIN was supplied, this complaint is tied to a specific vehicle and not just a model-year cohort.

Individual complaints are consumer-submitted and unverified by NHTSA engineers — the agency's role at this stage is to collect, index, and make them searchable. What matters for federal action is the pattern: when many owners of the same TOYOTA RAV4 cohort independently describe similar forward collision avoidance: automatic emergency braking failures, defect investigators have grounds to open a PE and request manufacturer data. Complaint Description

My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Airbags were not activated. 911 was dispatched. Police report made. Only minor injury finger fracture and chest bruise. There was a severe frontal damage to the car. Insurance deemed it totaled due to frame damage. I also complained to Toyota. They hired a software engineer to investigate. Just received official letter from Toyota there weee no defects in their safety system. The issues are the airbags not being deployed at the time of impact and pre collision auto braking non existent. The impact on the wall according to the people inside the nursing she works felt like an earthquake inside. But our main concern is the auto brake feature did not function.
