Complaint Description

I WAS CHANGING THE LEFT FRONT TIRE BECAUSE THE TIRE HAD A NAIL IN IT. I USED THE TIRE JACK FROM THE VEHICLE. THE VEHICLE WAS RAISED AND THE TIRE WAS REMOVED AND THE JACK COLLAPSED. THE VEHICLE WAS PERFECTLY LEVEL IN A GARAGE WITH A CEMENT FLOOR. THE VEHICLE WAS IN PARK. I DID NOT SET THE EMERGENCY BRAKE OR BLOCK THE TIRES. NOTHING WAS TOUCHING OR ROCKING THE VEHICLE AT THE TIME OF THE INCIDENT. LOOKING AT THE JACK AFTERWARDS, THE SCREW BOLT WAS BENT AND THE UPPER ARMS WERE BENT. *TR
